Help with simplest desighn

Hello all.

I am trying to build a simple app which will send an email with a content  based on user's selection.

For example: User selects a Category >> Sub Category >> Sub Category1, Enters his email and submits it

Than  a request will be send to an email address.

Request is just some text which will be created based on users selection.

Any help highly appreciated
