what app can my employees type their job info into and email it to me'

i am creating an app for my employees to do thier jobsheets on. before I commit to a purchase when I use the test app to my phone I can not enter any info into the jobsheet I created within the app can anyone tell me or help me out on what app i need to make. This is going to be a password protected app
