Need to email forms

I am in the process of creating an app for my company that will allow people in the field to submit forms via the app, thru an email link.  It has to include the form on the screen in the email.  Anyone know how to do that?

  The custom form widget works poorly, or I am doing it wrong... any reason ALL of the fields of the form are being inserted into the subject line?
