Cannot use "contacts" widget

I have the contact widget and want to use it in a 2nd app but just keep getting this error screen that runs indefinitely...

  Please start a support ticket with further screenshots and a description of how you stumbled into the issue. Any information you provide could expedite a fix to this problem.

  I started over with another app and it worked ok
