Custom form on Ipad

I am looking to publish short ebooks with note-taking capabilities. Is it possible to have the 'custom form' type of page on the ipad build?

Ideally, I would like to have the picture at the top, Title at the side of the page (like it is currently) the text content (like it is currently), and then a form/comments ability at the bottom for Q+A study, with a button to send the user's newly written notes to email, or note-recording program such as in the IBuildApp phone beta 'custom form' capabilities.

Any ideas? thanks for your help.

  Tim, ipad new templates and functions will be released in February.
  sorry we are short of man power as support of 58 000 community takes half of our time.
